DISCOVER DEBIT CARD
I understand that I am the only individual authorized to use the Discover Debit Card and that use of the card signifies agreement to the terms and conditions set forth in the Electronic Funds Transfer Disclosure. I also
understand that upon receipt of my Discover Debit Card I may call Community Financial Services Bank, (herein referred to as CFSB), Client Service Center 270-527-4600 or I may go to any CFSB ATM or by logging on to my Online Banking Account to activate the card.

Everything that I have stated in the application is correct to be the best of my knowledge. I hereby authorize CFSB to obtain a credit report and I further authorize CFSB to respond to any inquiries concerning my credit history. I understand I may be notified by a third-party vendor on behalf of CFSB if there is a question about a charge on my Discover Debit Card.

OVERDRAFT PROTECTION SWEEP
Automatic sweeps occur in increments of $50; however, the program will sweep all funds from your account down to a balance of $.01 if necessary.

ONLINE BANKING / DATALINK
For security reasons, account numbers will not appear on your computer screen at home. In the account Pseudo Name area, list accounts similar to the following examples: joint checking, his checking, car loan, home loan, etc. If you have questions, please contact Client Service Center at (270) 527-4600.

The undersigned acknowledges, having received, reviewed, and now agreed to the terms and provisions of CFSB, Online Banking Privacy Statement, and acknowledges that any use of same is restricted to persons authorized by CFSB to use same. I acknowledge application for account access and funds transfer using CFSB's Datalink 24 telephone transfer system and/or Online Banking Systems.

The undersigned also agrees not to share the user identification number and password assigned to them with a third party, and that no third party shall be authorized to conduct online banking/telephone transfers for them, and they agree to protect and hold CFSB harmless from any and all such transactions,
unless such transaction occurs as a direct result of CFSB's gross negligence or willful misconduct. It is the duty of the undersigned to protect any user identification number and/or password assigned to them by CFSB with that
degree of care required of all other financial accounts access codes issued to them by CFSB.

ESTATEMENTS
I hereby request that my bank statement for the account(s) listed on the application be changed from a paper (mailed) statement to an electronic statement with notice of delivery to be emailed to the listed email address. I understand this change will be effective upon CFSB's receipt of this authorization.

ONLINE BANKING PRIVACY STATEMENT
Community Financial Services Bank (CFSB) is committed to our customer's right to financial privacy in all transactions, in person, through mail, over the telephone, and at our website. This commitment to privacy has resulted in the establishment of this Privacy Statement.
CFSB (hereafter referred to as the bank) continually strives to develop products customized to the needs of our customers based on information gathered from our customers. The bank collects information from website
customers through the use of online applications, an optional guest book, a "contact us" feature, and by requesting e-mail addresses on applications for our "Online Banking" service. This information will be used to ascertain the credit and deposit needs of our customers and to offer suitable products to our online customers. Only that information necessary to complete a customer-initiated credit transaction, deposit account request, disclosure request or prequalification will be requested from online customers. Customers will be permitted to remove themselves from marketing efforts using an "opt out" feature provided with each marketing notice. Additionally, "cookies", small bits of information, are used by the bank's website host, Jack Henry & Associates, to verify the authenticity of each user name and password entered to access the Online Banking feature of the bank's website. Therefore, you may be unable to access certain portions of Online Banking if your computer defaults are set to refuse "cookies". The bank does not attempt to monitor any information or web surfing habits of our customers through the use of cookies. The bank makes disclosures to other third parties as permitted by law or requested by you.

Because the security of electronic mail cannot always be assured, confidential information such as account numbers and balances, personal identification numbers (PINs), credit limits, etc. will not be supplied through email. The security of information entered during on-line transactions is secured through the use of firewalls and encryption to prevent unauthorized access. Information obtained through on-line banking activity, including bill payment, does not disclose account numbers, personal identification numbers, or passwords. Customer information provided during on-line transactions will be regarded with the same degree of confidentiality as information obtained from any other source.

Bank customers are encouraged to notify the bank with any changes to information. Prompt attention will be given to any notifications with appropriate changes immediately. Customers may call the bank toll-free at 1-888-226-5669 or use the "contact us" feature on our website. Emails received from customers along with bank responses are saved and periodically audited to ensure compliance with Regulation E timing requirements. At CFSB, customer privacy will not be sacrificed for emerging technology. As we develop new products and services utilizing changing technology, we will continue to maintain high standards of information confidentiality within our organization.
